Please note that the Régie de l’assurance du Québec does not cover the renewal of medication without a medical visit and that fees may apply.
To obtain the result of a laboratory or radiology test (requested by your doctor), please complete the form below.
Please note that there may be a time delay before your result is available and also for processing your request (you will be contacted by phone). If this concerns an urgent test or one that worries you, contact the secretary at 450-455-9301 option 3 then option 6.
If you have a form that needs to be completed by your doctor, please drop it off at the front desk after making sure that it is signed. Please note that fees will be charged for forms that are not covered by the Régie de l'assurance maladie du Québec. Since delays may occur, it is best to bring your form as soon as possible. You will be contacted when it is completed.
To obtain a referral to a specialist or other health care professional, you must make an appointment with your family doctor to discuss the problem requiring a referral. A referral request for a problem that has not already been discussed with your doctor could be denied.
